Post-discharge alumni operations

After discharge, the relationship goes dark. Keep it running.

afterflow is the post-discharge engagement and retention operating layer for behavioral-health and addiction-treatment programs. One workspace for alumni follow-up, outreach, check-ins, review and referral asks, and outcome reporting after a person leaves treatment.

45 minutes, on your real workflow. No sales deck.

The post-discharge operating loopRuns every week
  1. Step 1: Roster

One record per alumni member: discharge date, sobriety date, owner, last contact.

  2. Step 2: Milestone touch

30, 60, 90 day, six month, one year, and multi-year follow-up, drafted and approved.

  3. Step 3: Check-in

Private self-report survey on mood, cravings, housing, work, and meetings.

  4. Step 4: Signal

Quiet members and concerning scores surface in a ranked at-risk queue.

  5. Step 5: Outreach

A coordinator reaches out with context, then logs channel, outcome, and next step.

  6. Step 6: Report

Sends, responses, review asks, referrals, and outcomes roll up for leadership.

Report feeds the next week's roster reviewthe loop repeats for as long as the program stays in touch.

The problem

The program loses visibility the day someone leaves.

Treatment is organized around 30 to 90 days. Recovery runs for years. Once a person discharges, the relationship, the follow-up, the outcomes, and the downstream value of that relationship all depend on whoever remembers to reach out. That is not a community problem. It is an operations gap.

  • Follow-up lives in a spreadsheet, a group chat, and one coordinator's phone.
  • Nobody can say who went quiet last month, or who owns reaching out.
  • Milestone dates pass unmarked because the calendar is a person's memory.
  • Reviews and alumni referrals happen by luck, not at the right moment.
  • Leadership cannot see post-discharge outcomes, so the program is funded on faith.

Lifelong

The alumni lifecycle afterflow is built around. Each mark is a scheduled touch, not a hope.

What retention means here

Staying in a working relationship with the people you discharged: reachable, checked in on, and known to the team. When someone needs a next step, the door is already open. afterflow tracks re-engagement outreach and alumni-referred admissions. It does not track or predict readmission.

Who runs this work

Four roles, one shared queue.

afterflow is built for the people who own post-discharge follow-up and the leaders who have to answer for it.

Alumni coordinators

“Who do I contact today, and what did we last say to them?”

Outreach and community leads

“Who went quiet, and which review or referral ask is due?”

Treatment-center operators

“Is post-discharge follow-up actually happening, or just intended?”

Behavioral-health executives

“What does our alumni program return in outcomes, reviews, and referrals?”

The recurring work

Six operational jobs, every week.

For each job: what the operator has to do, where it breaks today, and what afterflow supports right now. Nothing here is on a roadmap.

Maintain the relationship record

In the product today
The operator's task
Keep one accurate record per alumni member: discharge and sobriety dates, contact preferences, owner, tags, and last contact.
Where it breaks today
Records fragment across intake exports, sticky notes, and personal phones. Do-not-contact requests get lost.
In afterflow
Alumni roster with CSV import and export, confidential fields shown as initials plus a code, saved filters, bulk actions, and do-not-contact handling that holds every automated send.

Keep milestone follow-up on schedule

In the product today
The operator's task
Reach every alumni member at 30, 60, 90 days, six months, one year, and each year after, in a tone that fits their track.
Where it breaks today
Anniversaries are tracked by hand. The 90-day message goes out late, or not at all, and no one can prove either way.
In afterflow
A milestone timeline generated from the sobriety date, track-aware drafts a coordinator approves before they email, and a log of what went out and when.

Coordinate outreach when someone goes quiet

In the product today
The operator's task
Notice silence and concerning check-in scores early, decide who reaches out, and follow through.
Where it breaks today
Quiet is invisible until a crisis call. Outreach depends on whoever happened to notice, and the next step is never written down.
In afterflow
A ranked at-risk queue built on transparent, staff-tuned rules, an outreach draft with context, quiet-hours guardrails, and a logged next step with a due date. Signals support outreach decisions and are never a diagnosis.

Record meaningful engagement

In the product today
The operator's task
Capture every touch, response, and check-in so the relationship has a history the whole team can read.
Where it breaks today
Contact happens, but the record stays in one inbox. When staff change, the relationship history leaves with them.
In afterflow
A per-member activity timeline, private check-in surveys on mood, cravings, sleep, housing, work, and meetings, and an outreach log covering every send attempt, including the ones held back.

Run review and referral asks

In the product today
The operator's task
Ask for a public review or a warm referral at the right moment, then attribute what comes back.
Where it breaks today
Asks go out cold, or never. Alumni-driven admissions are remembered as anecdotes, not counted.
In afterflow
Review asks emailed with a private capture link, lower ratings routed to private feedback instead of a public site, per-member referral links, and referral status tracked from intro to admitted.

Understand post-discharge outcomes

In the product today
The operator's task
Show leadership what the alumni program does: sends, responses, outcomes, review velocity, referral attribution.
Where it breaks today
Reporting is a quarterly scramble through three tools. Outcomes after discharge are unknown, so the program is funded on faith.
In afterflow
A Reports screen with activity trends, engagement bands, outcome averages, review and milestone counts, and referral attribution, plus a save-rate view after at-risk interventions.

Said plainly so the fit conversation starts honest. Ask about timing in the assessment.

  • In-platform texting

Milestone, check-in, and review emails send from afterflow. Text messages and calls open on the staff member's own phone, with the draft prefilled.

  • Native bedflow connection

afterflow runs on its own and does not require bedflow. There is no data connection between the two products today.

  • EHR, EMR, or CRM integrations

Rosters move by CSV import and export. afterflow is not a clinical record and does not sync with one.

From engagement to evidence

Engagement, retention, reviews, referrals, and outcomes are one chain.

Each link depends on the one before it. afterflow makes every link a recorded step with an owner, so the program can see where the chain holds and where it breaks. It does not guarantee the numbers at the end of it.

  1. 1

    Consistent follow-up

Milestone touches and check-in requests go out on schedule and get logged.

  2. 2

    Visible signals

Responses, silence, and concerning scores surface in one queue with an owner.

  3. 3

    Timely asks

Review and referral requests are sent at moments of stability, not at random.

  4. 4

    Attributed results

Reviews, referrals, and outcomes are recorded against the members and campaigns behind them.

Where afterflow sits

Upstream is bedflow. Post-discharge is afterflow.

Three distinct identities from the same studio. afterflow is not a bedflow feature, and it is not a consulting engagement.

Upstream

bedflow

Referral operations, admissions coordination, and live bed visibility before and at intake.

Post-discharge

afterflow

Alumni engagement and retention operations after a person leaves treatment: follow-up, outreach, check-ins, reviews, referrals, outcomes.

Infrastructure

Solvhaus

The operational infrastructure and implementation partner behind both products, working with programs on the day-to-day operations side.

Alongside the tools you already run

afterflow is designed to complement the systems a program already has. Where there is no connection, it says so.

  • Clinical systems

Stay the system of record for clinical documentation. afterflow does not connect to them today and never holds a chart.

  • Spreadsheets

Bring the alumni list in by CSV and take it back out by CSV. The spreadsheet stops being the place follow-up lives.

  • Inboxes and phones

Milestone, check-in, and review emails send from the platform. Texts and calls still start from the staff member's own device.

  • bedflow

Runs upstream of afterflow. No native data connection between the two today; the products are designed to complement each other.
